Exercise Induced
Pulmonary Hemorrhage (EIPH)
Recent studies have
shown that approximately 90% of horses suffer from EIPH but in
98 percent of bleeders, bleeding is not noticeable. The most common indicator
for EIPH is bleeding from the nose during heavy training or competition,
but this is not always the case. When a horse bleeds, it usually starts from the
back of the lungs and when a horse is scoped, if there is blood found in the wind
pipe, it means that the horse has bled significantly.
There are several
theories regarding possible causes of EIPH in horses. The general consensus is
that if a horse has pre-existing problem such as respiratory ailment or if the
horse has had pneumonia at young age, then under the stress of racing, the blood
vessels in the lungs rupture. The newest research done in UK states that some
horses have a higher impact when running or exercising and this causes a ripple
in the chest cavity. Other possible causes include, horses running in races that
are too fast for their ability, genetics, over use of track drugs causing damage
to internal organs, toxins accumulated in the body, unhealthy lifestyle,
dependence on processed feed and little or no access to green grass. The liver
is also often involved in the toxic conditions causing lung problems, further
complicated by storage of track drugs and other poisons. Once horses have bled
from the lungs, this may cause scar tissue, infections and mucous build up which
may make it likely that the horses will continue bleeding throughout their
racing careers. But for a horses that has persistent lung infection, their lungs
may come back to being normal once the viral infection has healed.
A lot of Bleeders can be
helped by treating the inflammation in the lungs. The point to note is that
blood in the lungs will also cause inflammation; so the process can really be a
vicious cycle. Inflammation in the lungs can be treated with corticosteroids and
bronchial dilators. Giving horses supplements that are rich in bio-flavonoids
will also help.
Some
of the signs that link poor performance and other behavior to bleeding:
1.
History of one or good performance, followed by loss of form in subsequent
competition.
2.
Horse unable to sustain its all out speed, losing momentum and dropping back in
the home straight, or when pressed hard.
3.
Horse gets its "tongue over the bit", or "chokes down" and loses stride-rhythm
when pressed hard in the run to the line.
4.
Horse makes abnormal or choking sounds during all out sustained exercise.
5.
Horse appears distressed and uneasy and repeated swallowing is observed within
15 to 30 minutes after a race.
6.
Horse has shown history of slow recovery and is reluctance to put effort into
exercise for 3 to 4 days after hard workout or race.
7.
Horse lacks vitality, alertness and appetite for a few days after racing.
8.
Horse displays a trickle of blood-tinged mucous or there is a trace of blood on
the feed bin in the 24 to 36 hours following a race.
How to
manage EIPH
Reduce airborne dust to
decrease the risk of airway reaction and allergy. Dampen dusty feed including
chaff and hay, or if vegetable oil is used as energy source, mixing it well into
the feed will help reduce the dust. Select and maintain dust-free bedding such
as course saw dust, shredded paper, rice hulls or wood shavings. Dampen dusty
bedding with a fine mist spray and mix newly added top-up bedding well into
existing bedding. Work the horse out of line to the previous horse on dusty
tracks or arena surface. Avoid swimming of known or suspected bleeders,
particularly a horse that swims "hard". Avoid maximal, sustained effort and
blood pressure peaks in training by working at 3/4 pace with short hit-outs over
300 to 400m, 2 or 3 times a week, once the horse is conditioned for fast work.
